The forms provided below are required to be filled out as a precautionary measure and to assist the coaches in gaining a better understanding of any existing diagnoses or injuries that may affect your training experience. Your cooperation in completing these forms is greatly appreciated.
Dear NW Fitness Kids’ Parent/Guardian,
Thank you for allowing your child to be a part of the NW Fitness Kids Class! The commitment to our children’s future is a beautiful, arduous, amazingly nuanced path; the coaches are excited you have chosen NW Fitness Kids to be a part of that path as NW Fitness has changed all our lives immeasurably for the better. With your child, we are not only able to potentially enhance their lives, but we also can shape their lives and the realities within them. It is with this endeavour I have dedicated my life; your child’s safety and the opportunity to potentiate their personal lives are my primary focus.
I wanted to take this opportunity, now that the Kids Class is back and underway, to introduce myself formally and to set some policies and procedures for the safety of your child and the efficacy of their class experience.
My name is Mathew Tomkinson.
I currently work in a training environment as an operations director of Responsive Training Services Ltd, where seeing learners’ progress and development is an enjoyable experience. I served in the armed forces for 6 years where I travelled and gained valuable life skills, which I endeavour to share and teach to the younger generation. I would describe myself as a caring, fun and enthusiastic person. I also believe I have the skills and ability to inspire and encourage children of all abilities to reach their full potential. Great with children and a child at heart, I can come to their level and really influence a healthier lifestyle.
We run three groups for classes: Minis (ages 4–7), Littles (ages 8–11), and Teens (ages 12–17). Please note that age is not the only deciding factor for what class your child attends. It is largely based upon individual skill, athleticism, and maturity, and is ultimately decided upon by the head coach to ensure your child is getting the most out of the experience. Minis class lasts 20 minutes (10:15–10:35), Littles class lasts 30 minutes (10:45–11:15), and the Teens class lasts 60 minutes (11:30–12:30) and looks much like an adult class.
As previously stated, your child’s safety is paramount to the coaches, and it is with this focus I need your help. Below are some rules that we must enforce as a Community to best protect your child; please:
Minis parents must stay. Littles parents must stay.

I hereby permit images of my child, captured during regular and special activities, through video, camera and digital camera, to be used solely for the purposes of NW Fitness & NW Fitness Kids promotional material, publications and website, and waive any rights of compensation or ownership thereto. The last names of the child will not be given or posted on the internet or website.
Photo release is given on a per-parent basis when you book a trial, drop-in or block. You can change it at any time from the returning-family page.
